Document location

While figuring out how to promote the RM document to PR status, I noticed
that the process standards document was listed in all three areas (WD, PR
and REC). This could be confusing to anyone browsing the site.

I think it might be better to remove old references from the site, so a
document only can be found at its highest level. It is still possible to get
at old versions using the references in the header of the document.


Also, I thought the 'Latest Version' was supposed to have the form NAME.EXT
whereas it currently has TYPE-NAME.EXT (though the standard seems to have
lost this instruction from the Naming Convention - teach me to read changes
better) which means that this reference to a document will stop working when
a document is promoted. I'd like to suggest we use NAME.EXT.
