Personally, I think it would annoy me enough to coerce me into fixing things more often, thus being helpful.

I'm a bit confused on what you intend for the 'issues registry', perhaps because of your overloading the term. I think an actual registry would be a good deal of overkill, and combined with automatic email to the registry contact, a reporting page (with identifier/home registry/comment) much like what Mike Preciado has done for services would be more than sufficient. 

What do we do about records that require original publisher attention after X amount of time, or are not currently fixable (though I cannot currently think of any examples), or are otherwise disputable?

Of course, also we will all have to make sure that our registry contact information is correct. I know Space Telescope's lists Gretchen currently.
