Should the cursor/completeListSize attributes of resumptionToken elements be in the OAI-OMH namespace?

The OAI schema specifies the attribute 'attributeFormDefault="unqualified"'
which indicates that the attributes should *not* have namespace 
qualifiers.  Thus RofR and HEASARC are both correct.
