Prov-WD: Activity, Entity, ActivityConfiguration not formatted

Hi,

there are several places where Entity, Activity, WasDerivedFrom,
WasInformedBy, etc…, or ActivityConfiguration are not marked as class,
relation resp. package. The attached patch fixes this. There were also
some inconsistencies in using capitals (like wasDerivedFrom vs.
WasDerivedFrom) that are fixed as well.
