Just one minor gripe: When you fix Table 2 to have the base URL to be

  http://host[:port]/path?{name=[value]&}

we might want to make this

  scheme://host[:port]/path?{name=[value]&}

on gounds that the other parts of that, well, expression are
non-literals, too, and scheme is introduced in the paragraph above
the table as one of http or https.
