UPLOAD is a TAP feature and the uploaded table can be used in an ADQL query.
The position columns are explicitly taken from the name attribute of the
fields, so the caller has to use column names from the uploaded table: the
caller just uses them in the ADQL query as usual.

The PQL-specific issue is that the POS parameter will have to allow the caller
to specify both the name of the uplaoded table and columns. The draft for PQL
currently states that POS=@TAP_UPLOAD.sometable is used, but that is not
sufficiently explicit.

So, not an issue for TAP, nor for TAP+ADQL usage.
